  • Hypertension
  • Cigarette smoking
  • Obesity (BMI >/=30): indicates body mass index calculated as weight in kilograms divided by the square of height in meters
  • Physical inactivity
  • Dyslipidemia

  • Microalbuminuria or estimated glomerular filtration rate (GFR) <60 mL/min
